STROKE IN YOUNG ADULTS



Abstract
Background and Purpose: Cerebral stroke in young adults is relatively uncommon. The cause of stroke remains unknown i almost one-third of the cases, despite extensive diagnostic investigations. We studied to gain further insight into the pathogenic and etiologic determinats in young adults with stroke.
Methods: We retrospectively reviewed the medical records of 121 young patients aged 15~45 years who were admitted to the Kyung Hee Medical Center with the diagnosis of cerebral stroke from January 1994 to June 1996. We analysed clinical features and diagnostic test, such as brain imaging, cerebral angiography, echocardiography, 24-hour holter monitoring. Laboratory tests included complete blood cell count, serum glucose, lipid profile, rheumatologic profile, coagulation tests including fibrinogen, antithrombin-¥², protein C, protein S and antiphospholipid antibody.
Results: A total of 121 young patients with stroke aged 15-45 years(81 males and 37 females) comprised 9.8% of the 1,236 patients of all ages admitted for stroke. Of 121 patients, 87 atients(72%) showed ischemic stroke. The 87 patients with ischemic stroke were classified into; 1) large artery atherosclerosis(9 patients, 10.3%), 2) cardioembolism(10, 11.5%), 3) small artery occulusion(18, 20.7%), 4) other determined etiology(14, 16.1%), 5) undetermined etiology(36, 41.4%). The risk factors for ischemic stroke were hypertention(40 patients, 46%), diabetes mellitus(16, 18%), hyperlipidemia(34, 39%), previous stroke(22, 25%), smoking(16, 18%), alcohol(6, 7%), hypereosinophilia(4, 5%), protein S deficiency(2, 2%), protein C & S deficiency(1, 1%). The remaining 34 patiens (28%) had hemorrhagic stroke, which included nontraumatic and traumatic intracerebral hematoma, aneurysmal rupture, and vascular malformation.
Conclusion: Our results suggest that stroke of other determined etiology is a frequent causes of young age stroke. So, we should pay attention to these causes, such as nonatherosclerotic vasculopathies, hypercoagulabble state, or hematologic disorders.
